· Course Registration (Online) To be downloaded from www.fuoye-ng.com)
· Student Bio data Form (Online) ( To be downloaded from www.fuoye-ng.com)
· Attestation Letter
· Guarantor’s Letter 
· Affidavit of Good Conduct/non Membership of Secret Cult
· Certificate of Medical Fitness Obtained from University Health Centre
