Those who sat for the 2015 JAMB and scored 150 and above and chose Osun State Polytechnic Iree or intend to change to Osun State Polyechnic Iree can still obtain the screening form at OSPOLY Microfinance Bank Iree or OSPOLY Microfinance Bank, Ayetoro Osogbo or at the following branches of UBA : Iree, Osogbo, Dugbe Ibadan, Palm Grove Lagos, Onikolobo Abeokuta, Alagbaka Akure, Fajuyi Road and Ado-Ekiti
